CERTIFIED SERVICES
We provide the following certified services for Medicare, Medicaid, Blue Cross and PPOM:

  • SKILLED NURSING: A registered nurse (RN) will assess your needs, provide care and education, and coordinate all the therapies you may need.

  • HOME HEALTH AIDE: Certified aides can meet your personal care needs under the supervision of a registered nurse.

  • PHYSICAL THERAPY: Registered physical therapists (RPT) provide in home therapy to improve independence with walking, moving, exercise, and use of equipment such as walkers, canes, and wheelchairs.

  • OCCUPATIONAL THERAPY: Licensed occupational therapists (OTR) work with clients to improve independent living skills such as bathing, dressing and feeding.

  • SPEECH THERAPY: Licensed speech therapists (SLP) assess and treat speech, swallowing, and communication problems.

  • MEDICAL SOCIAL WORKER: Licensed social workers (MSW) provide education on community resources, short term counseling, assistance with long range planning and decision making.

CARDIAC RECOVERY PROGRAM

Health care trends are to reduce the length of stays in the hospital. Specialized cardiac nursing knowledge combined with a trained support team, clinical pathways and Hi-Tech equipment is the key to providing quality care while reducing medical cost. To meet the needs of the increasing number of people requiring cardiac care and education we have developed the Lutheran Home Care Agency Cardiac Recovery Program.

    • Restore patients with various cardiac disorders to active and productive lifestyles within the limitations imposed by their disease.

GERIATRIC CARE MANAGEMENT

The role of Geriatric Care Management (GCM) is to assist seniors and their caregivers in meeting immediate and long-term care needs. Our Geriatric Care Management services are provided by a Masters prepared Social Worker with over twenty years experience in mental health, home care, and hospice. Care managers provide a variety of services for seniors and their caregivers including:

    • In person care-planning assessments

    • Screen, arrange and coordinate in home help and services

    • Review financial and legal issues

    • Provide crisis intervention

    • Act as a liaison to families at a distance

    • Assist with transitioning to assisted living, adult foster care, or nursing home

    • Provide coordination and monitoring of medical care

    • Offer counseling and support

GCM's may also provide family or individual therapy, money management, and conservatorship or guardianship assistance. They may recommend assistance with transportation, shopping, yard management, accompaniment to physician offices, home care, homemaking services, or respite care.

This service can provide peace of mind to children who are trying to care for their parents at a long distance. It can be the voice of reason when multiple caregivers have varying ideas of what their loved ones could use. It can be a resource when a family member realizes their loved one needs some additional help, but they don't know what is available or where to find it.

GCM's have extensive knowledge about the costs, quality, and availability of services in their community.

The Lutheran Home Care Agency's Geriatric Care Management services are available to those residing within a 50 mile radius of Frankenmuth.

PRIVATE DUTY SERVICES

You and your family will be provided guidance on needed services that BEST meet your needs. Services are available up to 24 hours per day with a 2 hour minimum. Several different types of services are avialable through our Private Duty division of the Lutheran Home Care Agency:

WHICH PROGRAM IS FOR ME? CERTIFIED OR PRIVATE DUTY SERVICES?

  • CHOOSE CERTIFIED:

    • During times of illness, disability & recuperation, or continuing care after hospitalization when skilled care such as wound care, I.V. Therapy, cardiac monitoring, medication use & response, assessment, or teaching is required.

    • When chronic illnesses become uncontrolled requiring treatment changes. Examples: Diabetes, Heart and Lung diseases.
    • When there is loss of ability to manage an illness & caregivers must be taught how to provide the care needed to continue living in your home.
    • Costs are covered by Medicare, Medicaid, Blue Cross, and Other insurances - subject to individual benefit coverage.
  • CHOOSE PRIVATE DUTY:

    • When help is needed for personal care, everyday activities, and/or to asure safety in your home.

    • When household help, errand or transportation assistance, or companionship is required.
    • When a skilled assessment is needed to determine care approaches after there are medical, physical, or mental changes.
    • When case management is required to help you and your family manage difficult medical situations.
    • Costs are covered by private pay and some insurances.

PAYMENT FOR SERVICES:

Certified services are offered to the homebound person according to the physician's order. The Lutheran Home Care Agency is certified to bill Medicare, Medicaid, Blue Cross and other insurance carriers directly.

Private Duty services are paid for by some insurance carriers, but the majority is paid privately by the client.

HISTORY:
The year 2006 marks fourteen years of service by the Lutheran Home Care Agency staff to patients and their families in the Saginaw Valley and Thumb area. It's mission, to respond to the home care needs of people in the Saginaw, Bay, Tuscola, Genesee and Sanilac counties, has prompted the expansion of "Christ-centered" care through an array of services.

The certified home health division began providing skilled nursing, therapy, social work, nutritional and personal care services in 1992, becoming certified in 1993. People who are recovering from surgery, an acute illness, or exacerbation of a chronic illness receive these services to promote healing and independence.

Many regulatory and reimbursement changes in this area of home care have posed challenges to those providing home care. Determined to continue to keep these services available, LHCA staff have worked hard to not only survive, but thrive and expand services to meet the home care needs of our community.

In 1996 the Private Duty division's doors opened to offer patients individualized assistance needed to safely remain at home. Personal care, medication assistance, light housekeeping, companionship, meal preparation, and transportation and care management are the most commonly used services.

Now seven years old, the Hospice of Hope provides compassionate end of life support to people living with terminal illnesses. Hope, comfort and relief are the goals for each patient, family and loved ones. Approaches of skilled nursing, pain and symptom management, spiritual support, personal care, counseling, companionship, medical equipment and supplies are provided based on the person's unique needs and individual situation. Hospice allows each family to be free to enjoy the remaining time they have together, supports the meaning and purpose of each day and supports the dying process with dignity.

The staff of the Lutheran Home Care Agency is truly blessed to serve the people of our communities, realizing with gratitude, that if it weren't for the trust and backing of Lutheran Homes of Michigan, we would not exist. The year 2005, was filled with many accomplishments for the patients and staff of LHM's Home Care and Hospice Agency.

  • 2,129 people were served in 2005, representing an 8% growth over the previous year.

  • Lutheran Home Care Agency was chosen as a participant in a pilot apprenticeship program through the Paraprofessional Healthcare Insitute.

  • Over 35 children attended Camp Hope, a three day outdoor program for children who are dealing with grief and loss issues.
